OUR SERVICES

MEDICAID ALTERNATIVE PATHWAYS TO INDEPENDENCE (MAPs)

 

This service is available to anyone with an I/DD diagnosis who has graduated from high school or will graduate within three years. Supports are provided that gives our participants the tools, technology, and support they need to put them on a path to learn skills to help them work, live independently, learn about their neighborhoods, and find hobbies that interest them.

COMMUNITY-BASED

 

Supports are provided in the community that enable our participants to be valued active participants of their community. Community- Based participants are supported to engage in a variety of cultural outings and social enrichment activities, such as volunteering, educational classes, club membership, and attending events. Participants are supported in learning and practicing skills necessary for full inclusion in the person’s community, such as money management, socialization, or use of public transportation.

FACILITY-BASED TRAINING

 

Supports are provided at our facility that assist our participants to acquire, retain and improve skills in self-care, sensory/motor development, socialization, daily living skills, and communication.

EMPLOYMENT

 

Supports are provided that enable our participants to seek, gain and retain competitive employment. Journeys offers a wide range of employment services that include assessments, job training, job coaching and job placement.

SOCIAL SERVICES

 

Agency Case Managers provide supports to ensure our participants have access to their basic needs by assisting them acquire and retain needed benefits including, Social Security, SSI, Medicare, Medicaid, SNAP, housing assistance, unemployment, and eligibility.

HEALTHCARE OVERSIGHT

 

Supports are provided to assist our participants with their complex medical needs. Agency Case Managers oversees the scheduling of medical appointments, communication with physicians, acquiring treatment, and assisting with needed follow-up needed for each of our participants

RESIDENTIAL

 

Supports are provided that enable our participants to acquire, retain, or improve skills necessary to reside in a home of their own. Participants are supported in learning skills such as cooking, housekeeping, grocery shopping, and money management. Homes are owned or leased by the residents and are fully integrated into the community.

TRANSPORTATION

 

Supports are provided to assist our participants in getting to work, to their medical appointments, and to places they wish to go in the community.
